Salina Family Healthcare-Dental will be in the District in September to perform OPTIONAL dental screenings on your students. These screenings are performed by a Registered Dental Hygienist.
An additional FLUORIDE application, at NO COST to the student, can also be performed but will need a signed consent form due back by September 1st.
All District students will be screened unless there is a signed form by parent/guardian requesting to OPT OUT due back by September 1st. Do not sign and return the form if you want your child to be screened. Send it back only if you want to OPT OUT of the screening or Send it back if you WANT the additional fluoride treatment on top of the screening.
