Medications at School

Consent forms for Medications in the School Setting (a new form must be obtained for each new school year.)

Over the Counter Meds (OTC):  A consent form must be filled out and signed by a parent / legal guardian.   Medications must be brought in by the parent / legal guardian and not sent with the student.  Medications must be brought in the original container / packaging with a good expiration date.   https://core-docs.s3.amazonaws...

Prescription Medications:  A consent form must be filled out and signed by a parent / legal guardian AND signed by the ordering physician BEFORE medications can be given at school.  Medications must be brought in by the parent / legal guardian and not sent with the student. Medications must be brought in the original prescription bottle / package that is labeled specifically for that student with a good expiration date. Self Carry / Self Administer Meds (inhalers, epi pens) (Ellsworth Junior / Senior High School):  A consent form must be filled out and signed by a parent / legal guardian AND signed by the ordering physician that gives the order to allow that student to self-carry these medications.                         https://core-docs.s3.amazonaws...
