It is my joy and pleasure to work with your student, becoming his/her school health advocate. I encourage all parents and students to contact me, Nurse Bonnie Peterman, with any health-related questions or concerns. Together, we can improve and maintain our students' body, health and well-being.

Immunization Requirements

Immunizations are required for any individual who attends school. For that list, click "School Bus" under Health Office Documents. There are exceptional circumstances that could alter the required doses for a child, but please talk with your primary care provider, Nurse Bonnie or the Ellsworth County Health Department.

Food Allergies

USD 327, in partnership with OPAA! Food, does their best to accommodate students with specific food needs.

A Meal Modification Consent Form must be completed by a medical professional (MD, DO, PA or APRN).

All forms must be submitted to Nurse Bonnie at bpeterman@usd327.org.

Medications at School

Whether your student requires prescription medication or over-the-counter medication at school, Nurse Bonnie needs a Permission for Medication Consent Form, renewed yearly with each new enrollment year.

A medical provider must complete and sign the Consent Form when administering prescription medication, whereas a parent or guardian is only required to sign the Consent for over-the-counter medication.

Forms must be submitted to Nurse Bonnie at bpeterman@usd327.org or faxed to 785-472-8118.

Sick Child Attendance Policies

In an effort to maintain health and wellness among our student body, we require students to remain home if he/she has experienced any of the following ailments within the past 24 hours:

  • fever

  • vomiting

  • diarrhea

  • chills

  • sore throat

  • strep throat

  • pink eye

  • bad cold, with very runny rose or bad cough, especially if the child has been awake through the night.

If a child has been to the doctor, and was prescribed antibiotics, the student must have been taking it for 24 hours before returning to school.

Rule of thumb: any time your student goes to the doctor, ask their office to provide you a doctor's note dismissing the student from school for the duration of the appointment, or have them fax it to the EES Office at 785-472-8118.

Chronic Conditions

If your child has a chronic condition, together, with Nurse Bonnie and your primary medical care provider, you will create an emergency/rescue plan as well as an emergency action plan.
